Self-Pay Addiction Medicine Services

| Extended Follow-Up Visit For complex cases requiring additional time, multiple diagnoses, significant treatment changes, relapse evaluation, or extensive care coordination. | $250 |
| Telehealth Follow-Up Visit Secure virtual follow-up appointments available for eligible patients located in California and Nevada at the time of service. | $175 |
| Medication-Assisted Treatment (MAT) Included with Follow-Up Visits. Management of medications such as buprenorphine, extended-release buprenorphine, naltrexone, and other evidence-based treatments when clinically appropriate. | Included |

Financial Policies
Fees listed are current self-pay rates and are subject to change without notice. The fees charged for services are those in effect on the date services are rendered.
Patients are responsible for all charges associated with services provided, including office visits, procedures, laboratory testing, administrative services, and any applicable fees not covered by insurance.
Returned payments, disputed charges, chargebacks, and reversed credit card transactions may be subject to additional administrative fees and collection costs to the extent permitted by applicable law.
Patients initiating a credit card dispute or chargeback for services that were rendered may remain responsible for the original charges, administrative processing costs, and any fees incurred by the practice in responding to the dispute.
Laboratory testing, confirmatory toxicology testing, and services performed by outside entities may result in separate charges billed directly by the performing organization.
Good Faith Estimate
Self-pay patients have the right to receive a Good Faith Estimate of expected charges in accordance with applicable federal regulations.
Prescription Refill Policy
Prescription refill requests may require an appointment and are subject to physician review, clinical appropriateness, and applicable prescribing regulations.
